Health forms and Class B Order form link

All, 

I hope everyone has had a great week. As a reminder, we must collect Health forms for all summer camp participants (youth and adults).  Here is what we need: 

Youth: Health forms A/B1/B2/C  – a physician must sign form C.  Please don’t forget to sign the form with your scout and include a copy of an insurance card as well.  

Adults:  We need the same forms and a copy of your up-to-date YPT.  

Finally, we have a link to order Class B Scout shirts directly from 1st Class Design.  Below is the link to order.
